Please complete only one of the Tier options below. Completing more than one Tier option below will invalidate your claim.

Tier 1 Claim

Tier 1 Benefit is available for Settlement Class Members who purchased Products during the Class Period and do not have valid Proof of Claim. If you fill in the circle below, then you will receive $3.00 per Household.


Tier 2 Claim

Tier 2 Benefit is available for Settlement Class Members who purchased a single Product during the Class Period. Selecting Tier 2 requires certain documentation but allows you to recover up to $10.00 rather than the $3.00 available in Tier 1. You may recover 25% of the difference between the advertised “original” price on Wish.com and the lower “original” price from another retailer. So, for example, if the Wish.com “original” price was $100.00 and the “original” price from another retailer was $60.00, then you could receive the maximum $10.00 Tier 2 benefit ($100.00 - $60.00 = $40.00; $40.00 x 25% = $10.00). Similarly, if the Wish.com “original” price was $80.00 and the “original” price from another retailer was $60.00, then you could receive a $5.00 benefit under Tier 2 ($80.00 - $60.00 = $20.00; $20.00 x 25% = $5.00).

Your Tier 2 claim requires documentation of the following from you: (1) proof of the “original” price at which Wish.com advertised the item you purchased at the time you purchased it; and, (2) proof of the “original” price from another retailer (online or bricks-and-mortar) advertising the identical item you purchased from Wish.com within 90 days of the date you made your purchase. Examples of “proof” may be receipts, screen shots, newspaper advertisements, etc.

If you are not able to provide proof of either (1) or (2), then you can provide a statement, under penalty of perjury, detailing why you could not find the required documents. The Settlement Administrator (not Wish.com) will determine whether your statement adequately explains the reason you could not provide the required proof. The Settlement Administrator may take into account whether your statement is notarized.


Tier 3 Claim

Tier 3 Benefit is available for Settlement Class Members who purchased multiple Products during the Class Period. Selecting Tier 3 requires certain documentation but allows you to recover up to $20.00 rather than the $3.00 available in Tier 1 or the $10.00 available in Tier 2. You may recover 25% of the difference between the advertised “original” prices on Wish.com and the lower “original” prices from other retailers. So, for example, if the Wish.com “original” prices on the items you purchased add up to $140.00 and the “original” prices from other retailers for the identical items add up to $60.00, then you could receive the maximum $20.00 Tier 3 benefit ($140.00 - $60.00 = $80.00; $80.00 x 25% = $20.00). Similarly, if the Wish.com “original” prices add up to $110.00 and the “original” prices from other retailers add up to $60.00, then you could receive a $12.50 benefit under Tier 2 ($110.00 - $60.00 = $50.00; $50.00 x 25% = $12.50).

Your Tier 3 claim requires documentation of the following from you: (1) proof of the “original” prices at which Wish. com advertised the items you purchased at the time you purchased them; and, (2) proof of the “original” prices from other retailers (online or bricks-and-mortar) advertising the identical items you purchased from Wish.com within 90 days of the date you made your purchases. Examples of “proof” may be receipts, screen shots, newspaper advertisements, etc.

No alternative statement shall substitute for the required documentation in Tier 3.

You are submitting to the jurisdiction of Cole County, Missouri.

Certification under Penalty of Perjury
  1. I have read the Settlement Agreement and agree to its terms, including the Release(s);
  2. The information provided in this Claim Form is accurate and complete to the best of my knowledge, information and belief;
  3. The additional information provided to the Settlement Administrator to support my Claim is an original or a complete and true copy of the original document;
  4. I am a member of the Settlement Class and did not request to Opt-Out from the Settlement Class;
  5. I have not entered into a Settlement for any of the Claims set forth in this Claim Form;
  6. I am neither (a) a Person who purchased or acquired the Product for resale; (b) an employee, principal, legal representative, successor, or and assign of Defendant or its affiliated entities; (c) a government entity; nor (d) a judge to whom this Action is assigned, or any member of the judge’s immediate family;
  7. I have not submitted any other Claim for the same purchases and have not authorized any other Person or entity to do so, and know of no other Person or entity having done so on my behalf;
  8. No other Person in my Household has submitted a Claim under this Settlement;
  9. I will timely provide any additional information requested by the Settlement Administrator to validate my Claim;
  10. I understand that by submitting this Claim Form, I am deemed to have given a complete Release of all settled claims; and
  11. I understand that Claims will be audited for veracity, accuracy and fraud. Illegible Claims Forms can be rejected. If a Claim Form is determined not to be a Valid Claim, it will be rejected.
