During this online process you will have the opportunity to upload your supporting documentation. Uploaded documents must be in PDF format only and the maximum file size per document is 10 MB. You must respond to all questions when submitting your claim online. Incomplete Proof of Claim Forms will not be accepted. You must select "Submit Claim” when you reach the end of the online process, or your information will not be saved. Your confirmation page also contains important instructions, such as how to submit your supporting documentation in the event you were not able to provide upload it electronically.
